About The Position

This role involves coordinating social services within Charter House. The position requires a Bachelor's degree in Social Services or Social Work, or a nursing background with specific licensing and experience. The coordinator must be able to collaborate effectively, possess knowledge of Medicare and other third-party payors, and demonstrate strong communication, human relations, and problem-solving skills. Adaptability to unpredictable situations is also essential.

Requirements

A Bachelor's degree from an accredited college or university in Social Services or Social Work.
Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program.
If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.
Must be able to work in collaboration with others.
Is knowledgeable about Medicare and other third-party payors.
Requires good communication and human relation skills.
Strong problem-solving skills are necessary with the ability to collect, compare, sort and prioritize information to be used in the decision-making process.
Must be able to adapt to unpredictable situations.
Prior experience working with a group program or organized recreational activities.
Requires a license by the Minnesota Board of Social Work at minimum a LSW (Licensed Social Worker) or MN Licensed Practical Nurse license.
One year of work experience in Long Term Care or Home Health is required.

Experience working with the geriatric population is preferred.
