About The Position
The Social Services Assistant works with the Social Services Director/Coordinator to identify and provide for each resident’s social, emotional, and psychological needs and support the continuing development of each resident’s full potential during their stay. The role also assists with discharge planning. Green Valley Rehabilitation Center is part of the Volare Health portfolio of long-term care skilled nursing and short-term rehabilitation centers. The stated pay range is $21.67–$26.26 per hour, depending on experience.
Requirements
- Meet any applicable state and/or federal requirements.
- Have a basic understanding of computer technology.
- Be able to read, write, and follow oral and written directions in English.
- Relate positively and favorably to residents and families and work cooperatively with other employees.
Nice To Haves
- Bachelor’s degree in social work or a related field (preferred).
- Healthcare experience is highly preferred.
Responsibilities
- Develop comprehensive social history and psychosocial assessments, discharge plans, and complete the MDS and Care Plan for new admissions.
- Orient and assist new residents in adjusting to the facility.
- Assist residents and families with adjustment to changes, including room changes.
- Assist families with engraving and/or marking residents’ personal items.
- Review residents’ rights with residents and staff and attend to residents’ various personal needs.
- Assist in developing a volunteer program and facilitating family group meetings.
- Document progress notes related to each resident’s care plan when necessary and within the policy timeframe.
