About The Position

The Social Services Assistant works with the Social Services Director/Coordinator to identify and address each resident’s social, emotional, and psychological needs and support the continuing development of residents’ full potential during their stay at the facility. The role also assists with planning for residents’ discharge. Green Valley Rehabilitation Center is part of the Volare Health portfolio of long-term care skilled nursing and short-term rehabilitation centers. The position pays $21.67–$26.26 per hour, depending on experience. The employer describes opportunities for advancement and a rewarding career.

Requirements

Bachelor’s degree in social work or a related field (preferred).
Health care experience is highly preferred.
Meet applicable state and/or federal requirements.
Basic understanding of computer technology.
Ability to read, write, and follow oral and written directions in English.
Ability to relate positively and favorably to residents and families and work cooperatively with other employees.

Nice To Haves

Bachelor’s degree in social work or a related field.
Health care experience.

Responsibilities

Develop a comprehensive social history assessment, psychosocial assessment, and discharge plan, and complete the MDS and Care Plan for new admissions.
Orient new residents and assist them in adjusting to the facility.
Assist residents and families with adjustment to changes, including room changes.
Assist families with engraving and/or marking residents’ personal items.
Review residents’ rights with residents and staff, and attend to various personal needs of residents.
Assist in developing a volunteer program and facilitate family group meetings.
Document progress notes related to each resident’s care plan when necessary and within the policy timeframe.
