About The Position
Dulles Health & Rehab Center in Herndon, VA is searching for a dynamic Social Services & Care Coordinator to join their interdisciplinary team. This role is for an exciting, fast-paced, and challenging opportunity to use social work assessment skills and service focus to make a difference for patients.
Requirements
- High School Diploma or GED
- Strong customer service, teamwork and communication skills.
Nice To Haves
- Some training or experience in working in a healthcare social service setting preferred
Responsibilities
- Serve as an essential member to the interdisciplinary team in developing and maintaining a patient centered plan of care addressing patient/residents identified needs and verify all disciplines are addressing planned interventions to ensure a safe transition to the next level of care.
- Assist the Director of Social Services with the timely delivery of NOMNC & ABN Notices as well as the expedited appeals process.
- Complete all appropriate sections of the MDS timely and accurately.
- Assist the Director of Social Services with Quality Improvement efforts, and regulatory compliance as it relates to NOMNCs, PASRR, Advance Directives, Care Plan meetings, and Ombudsman notification.
- Complete appropriate referrals to community agencies and resources available to meet the needs of patients throughout care transitions.
- Demonstrate and promote a positive, collaborative, and caring service excellence team environment.
