About The Position

The Social Services Coordinator is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. Working under the supervision of a Licensed Master of Social Worker (LMSW) in hospice, the Social Services Coordinator collaborates with the hospice team to improve the quality of life for patients receiving end-of-life care. S/he follows the Plan of Care established offers emotional support, coordinates community resources, and communicates needs of hospice patients to the supervising LMSW.

Requirements

  • Bachelor’s degree in Social Work from an institution accredited by the Council on Social Work Education, or a Bachelor's degree in psychology, counseling, or related field and one (1) year of social work in a healthcare setting.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties.
  • Strong written and verbal communications.
  • Ability to understand, read, write, and speak English.
  • Articulates and embraces integrated healthcare at home philosophy.
  • Compassionate and empathetic approach to end-of-life care.
  • Knowledge of community resources and social services.
  • Clinical experience or training related to death, dying, and bereavement.
  • Strong organizational and time management skills.
  • Must be able to work a flexible schedule to include nights and weekends.

Nice To Haves

  • Previous experience in hospice, palliative care, or a healthcare setting preferred.

Responsibilities

  • Follows the established Plan of Care providing support to meet the emotional, social, and practical needs of patients and families.
  • Collaborates with the hospice team to support the development of individualized care plans tailored to the unique needs of each patient.
  • Provides emotional support to patients and their families, fostering a compassionate, and understanding environment.
  • Assists individuals in coping with grief, loss, and the emotional challenges associated with end-of-life care.
  • Identifies and coordinates community resources and support services to address the social and practical needs of patients and families.
  • Establishes and maintains relationships with local organizations to enhance the availability of resources.
  • Facilitates discussions around end-of-life expectations, ensuring patient preferences and goals of care are respected.
  • Collaborates with the hospice team to communicate and document patient preferences effectively.
  • Provides education to patients and families about available social services, community resources, and the hospice care process.
  • Advocates for the needs and preferences of patients and families within the hospice team and with external service providers.
  • Maintains accurate and timely documentation of social services directed by the Plan of Care.
  • Collaborates and provides regular reports to the LMSW regarding patient status, resource utilization, and other relevant metrics.
  • Supports psychosocial needs and emergencies, by providing emotional comfort and support to patients and their families.
  • Collaborates with appropriate agencies and authorities to ensure the safety and well-being of patients.
  • Performs other duties as assigned.
