About The Position

On Lok PACE is a non-profit organization dedicated to the care of elders through the Program of All-Inclusive Care for the Elderly (PACE). PACE was developed to address the challenges of caring for frail seniors, believing it is best for their well-being and their families to be served in their community. Seniors and their caregivers often face issues such as transportation, medication management, coordination of medical care, lack of social interaction, and the ability to remain at home. On Lok PACE participants receive in-home care services and transportation to an On Lok PACE center for primary medical care, social and recreational activities, and other senior care services. Requirements

  • AA degree, BA preferred, in related field of human services.
  • Minimum one-year experience working with the elderly or frail population required.
  • Ability to work independently and as part of multidisciplinary team.
  • Flexible and organized individual who takes initiative and is very detail-oriented.
  • Computer literate.

Nice To Haves

  • Bilingual in Spanish and/or Chinese, preferred.

Responsibilities

  • Assists the Social Workers in coordinating services for participants.
  • Provides administrative and clerical support to the Social Workers as delegated. Duties will include tasks such as assisting with completion of RCFE and enrollment conference packets, arranging transportation trips, booking appointments in the electronic health record, coordinating activities of daily living, escorting participants, providing assistance with translation and interpretation, assisting with Medi-Cal redetermination and other administrative support matters.
  • Acts as participants' advocate, spending quality time with participants in the day health center and encouraging them to participate in conversation and other activities. Provides reports and feedback to the Social Workers on a frequent basis.
  • May attend IDT meetings to provide additional support for participant care, enhancing the quality of care and daily operations as needed.
  • Documents in the electronic health record in a timely and accurate manner.
  • Complies with all On Lok policies & procedures including but not limited to PACE, safety, and confidentiality requirements.
  • Incorporates the Healthcare Hospitality G.R.E.A.T. Practices for 5-Star service.
  • May need to travel within the region as assigned.
  • Other related duties as assigned.
