About The Position

On Lok PACE is a non-profit organization dedicated to the care of elders through the Program of All-Inclusive Care for the Elderly (PACE). The PACE model focuses on serving seniors with chronic care needs and their families within their community. Participants receive in-home care services and transportation to a PACE center for medical care, social activities, and other senior care services. The organization seeks individuals to help improve lives and advance healthcare.

Requirements

  • AA degree, BA preferred, in related field of human services.
  • Minimum one-year experience working with the elderly or frail population required.
  • Ability to work independently and as part of multidisciplinary team.
  • Flexible and organized individual who takes initiative and is very detail-oriented.
  • Computer literate.

Nice To Haves

  • Bilingual in Spanish and/or Chinese, preferred.

Responsibilities

  • Assists the Social Workers in coordinating services for participants.
  • Provides administrative and clerical support to the Social Workers as delegated.
  • Assists with completion of RCFE and enrollment conference packets.
  • Arranges transportation trips.
  • Books appointments in the electronic health record.
  • Coordinates activities of daily living.
  • Escorts participants.
  • Provides assistance with translation and interpretation.
  • Assists with Medi-Cal redetermination and other administrative support matters.
  • Acts as participants' advocate, spending quality time with participants in the day health center and encouraging them to participate in conversation and other activities.
  • Gives reports and feedback to the Social Workers on a frequent basis.
  • May attend IDT meetings to provide additional support for participant care.
  • Documents in the electronic health record in a timely and accurate manner.
  • Complies with all On Lok policies & procedures including but not limited to PACE, safety, and confidentiality requirements.
  • Incorporates the Healthcare Hospitality G.R.E.A.T. Practices for 5-Star service.
  • May need to travel within the region as assigned.
  • Other related duties as assigned.
