About The Position

We are seeking a compassionate and dedicated Social Services Coordinator to join our team and play a vital role in supporting the emotional well-being and quality of life of our residents. In this position, you will develop supportive relationships with residents and families, help guide them through transitions, advocate for resident needs, and work closely with our interdisciplinary team to foster a warm, caring, and resident-centered environment. From welcoming new residents into our community to providing ongoing emotional support and care coordination, this role is centered on compassion, communication, and connection. Requirements

  • Bachelor’s degree in social work, human services, psychology, healthcare administration or related field preferred.
  • Previous experience in healthcare, long-term care, rehab or skilled nursing preferred.
  • Ability to communicate effectively, verbally and written, with residents, families, and staff.
  • Basic computer skills and working knowledge of Microsoft Office programs.
  • Ability to convey a sense of caring and respect to residents and families, regardless of circumstances.

Responsibilities

  • Supporting residents’ psychosocial well-being and quality of life
  • Building trusting relationships with residents and families
  • Leading welcome meetings and assisting with resident transitions
  • Advocating for resident rights and addressing concerns
  • Serving as the community grievance officer
  • Coordinating supportive services and resources
  • Participating in care planning and interdisciplinary collaboration
  • Assisting with durable medical equipment (DME) coordination and other resident needs
