This is a PT on-site opportunity working in our long-term care community. As the WCHS Social Worker, this position is responsible for assisting and supporting the residents, clients, and patients of WCHS by providing passionate expertise and personalized service, assisting residents, clients, and patients in making positive lifestyle changes, and providing resources to encourage emotional growth and stability. The Social Worker understands that every patient and resident is unique and worthy of compassion and of the very best care.

Requirements

  • Bachelor’s Degree in Social Work, Psychology, or Sociology preferred, 1 year of work experience, or an acceptable combination of both education and work experience.
  • Social Work License Preferred
  • Excellent communication skills, including the ability to explain medical concepts to residents, clients, and patients in a way that is easily understood.

Responsibilities

  • Promotes the mission, vision, and values of Weston County Health Services (WCHS).
  • Assess each resident, client, and patient’s unique condition, combining data from medical diagnoses, personal interviews, and professional knowledge to establish an accurate picture of overall mental and emotional health.
  • Develops comprehensive plans to help patients adjust to medical diagnoses, including recommending support groups and lifestyle changes, and helping residents, clients, and patient transition from hospital care to personal healthcare management.
  • Provides necessary support for each resident, client, and patient according to health status, personal strengths and weaknesses, emotional state, and additional factors.
  • Research local support groups and other resources to determine which ones are right for individual residents, clients, and patients, taking into account eligibility requirements and treatment methodologies.
  • Evaluate services and resources provided to residents, clients, and patients to determine effectiveness, and modify treatment plan and recommendations as necessary.
  • Follow up with residents, clients, and patients throughout diagnosis, treatment, and recovery to determine current state of mental and emotional health, provide required support and create relevant recommendations for changes to lifestyle and diet.
  • Maintain current knowledge of local resources and relevant medical research to provide best possible recommendations to residents, clients, and patients. Coordinates with medical staff and other departments to ensure efficiency.
  • Attends and participates in staff meetings and other assigned meetings.
  • Adheres to accreditation and compliance standard guidelines.
  • Assists in completing required reports, program reviews and preparation.
  • Utilizes good communication skills to maintain positive relationships with clients, providers, co-workers and other sections in the organization and with other facilities in the community.
  • Maintains a professional, organized and clean working environment by following organizational policies, guidelines, and safety standards.
  • Completes Work Assignments in a timely manner.
  • Assists visitors with directions whenever requested.
  • Always introduces self to patients, residents, families, and visitors when appropriate.
