About The Position

Accountable for enhancing the resident's quality of life providing direct social work service and counseling to residents and families. This role involves coordinating admissions and discharges, formulating psycho/social/population specific and cultural needs plans, and providing resource information to residents and their families. The Social Services Designee also communicates and cooperates with the multi-disciplinary care team, completes necessary documentation, and assists with resident personal needs such as ordering clothing and shopping. They provide back-up support to other departments and participate in Quality Improvement activities.

  • Ability to hear, understand and distinguish speech and other sounds.
  • High School or GED Equivalent

Responsibilities

  • Enhancing the resident's quality of life providing direct social work service and counseling to residents and families.
  • Visiting with families, residents, prospective residents/families inquiring about facility, giving tours, and referring to Department of Social Services for financial assistance and providing follow-up.
  • Coordinating admissions and discharges for long term care.
  • Notifying all departments and parties involved, obtaining necessary information.
  • Formulating plans to meet psycho/social/population specific and cultural needs, attending at risk meetings and including residents and families when formulating plan.
  • Providing resource information to residents and their families.
  • Communicating and cooperating with multi-disciplinary care team regarding resident care in the skilled and long term care setting documenting assessments, care plans, and MDS assessments in an accurate and timely manner.
  • Completing denials and expedited reviews.
  • Assisting in cleaning, organizing and maintaining resident dresser drawers and closets regularly.
  • Ordering resident clothing as needed and shopping for residents as needed.
  • Providing back up support to activities and skills rehab social services.
  • Prioritizing and organizing assigned workload to facilitate completion of work in a timely manner.
  • Scheduling care conferences and appointments with residents and families.
  • Attending patient care conference, writing care plans, and explaining care plans to residents/families as requested.
  • Participating in Quality Improvement activities and having knowledge of the department’s QI indicators.
