About The Position

Baywood Court is seeking a Social Services Designee for its Health Center (Skilled Nursing Facility). This role is crucial in supporting residents and their families by addressing psychosocial needs, facilitating transitions into the care environment, and ensuring comprehensive discharge planning. The Social Services Designee acts as an advocate and resource, promoting resident dignity, well-being, and continuity of care. This position involves collaborating with residents, families, and the interdisciplinary care team to develop person-centered care plans, navigate emotional and social challenges, and ensure compliance with state and federal regulations (Title 22 and CMS). The role is ideal for a compassionate professional passionate about making a positive impact in the lives of seniors.

Requirements

  • Must possess Social Services Designee Certificate
  • Proficiency with computers and working knowledge of Microsoft Office applications
  • Knowledge of SNF regulations (CMS, OBRA, Title 22)
  • Strong assessment and care coordination skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Compassion, empathy, and strong advocacy mindset
  • Knowledge of community resources and discharge planning processes
  • Conflict resolution and problem-solving skills
  • Ability to read, write and speak English clearly using proper speech and grammar.
  • Ability to communicate clearly, accurately, tactfully, and effectively with employees, residents, families and physicians while maintaining a professional demeanor.

Nice To Haves

  • Bachelor’s degree in Social Work or a related human services field
  • Licensed Social Worker (LCSW) or Associate Clinical Social Worker (ACSW)
  • MatrixCare proficiency
  • Experience in a skilled nursing facility or long-term care setting
  • Bilingual (e.g., Spanish, Mandarin, etc.) depending on resident population

Responsibilities

  • Conduct psychosocial assessments for residents upon admission, quarterly, annually, and as needed.
  • Participate in interdisciplinary care plan meetings and contribute to individualized care plans.
  • Identify psychosocial, emotional, and discharge planning needs of residents.
  • Provide counseling, emotional support, and crisis intervention to residents and families.
  • Facilitate communication between residents, families, and facility staff.
  • Address resident concerns and grievances in accordance with facility policies.
  • Coordinate safe and timely discharge planning, including referrals to community resources.
  • Facilitate transitions of care to hospitals, home health, assisted living, or other settings.
  • Connect residents and families with community resources such as financial assistance, transportation, and mental health services.
  • Advocate for residents’ rights and ensure respect, dignity, and self-determination.
  • Ensure compliance with federal (CMS), California Department of Public Health (CDPH), and Title 22 regulations governing social services.
