Requirements

  • BA in social work or related field required.
  • Experience providing case management related services to mentally ill and/or substance abusing individuals required.
  • Effective interpersonal communication and organizational skills, and staff supervision experience required.
  • CPR training certification or willingness to take training class in CPR.

Nice To Haves

  • Knowledge of AWARDS and Yardi preferred.
  • Bilingual in English and Spanish is a plus.

Responsibilities

  • Provide direct social services and resident management related functions to tenants.
  • Perform case management services including intake and admission related activities.
  • Assist tenants with a full range of needs such as mental health and substance abuse interventions.
  • Refer and coordinate with primary health and behavioral health providers.
  • Provide financial management assistance.
  • Make health and safety referrals.
  • Conduct regular and as needed apartment visits to ensure safety and security.
  • Prepare and maintain case records including individualized care plans and service notes.
  • Work in conjunction with the resident manager regarding tenant payment, eviction proceedings, management of front desk staff, and coordination of facility contractor activities.
  • Provide emergency first aid/CPR and/or Narcan assistance when needed.
  • Perform related duties as assigned.
