Position Description: The person in this position acts as a client advocate for and provider of needed early intervention services for children 0-3 years of age and families meeting the Early On criteria. Core elements of this position include assessment, service coordination, and developmental screening, service plan development, providing developmental interventions, re-assessment and follow-up and monitoring services. Services are provided in the home. The person in this position works independently under the general guidance and supervision of the Early On Coordinator. The person in this position will split 60% of their time doing this role and 40% of their time with data entry and support work with program needs.

Requirements

  • Minimum of a Bachelor's Degree in Early Childhood Development, Social Work or other related field.
  • Associate's Degree can be accepted with a crediting internship
  • Early childhood experience and/or knowledge regarding infant/toddler development required
  • Excellent communication, assessment and intervention skills
  • Ability to work with a wide variety of families
  • Valid MI license and vehicle for home visits

Nice To Haves

  • Early On experience not required; but preferred

Responsibilities

  • Timely documentation of all activities and maintaining Early On files according to compliance standards
  • Support coping skills in parenting/education regarding developmental delays/disabilities
  • Ensure assessments, Individual Family Service Plans (IFSP), screening and coordination of services between schools, healthcare providers, human services, and mental health
  • Facilitate Individual Family Service Plans (IFSP) meetings
  • Act as client advocate
  • Provide two monthly home visits, developmental interventions and activities for parents and children
  • Meet with the Early On Manager/FSC on a regular basis and attend any required trainings
  • Must meet agency productivity/performance standards
  • Complete MISTAR data entry in the Michigan's State system for the program.
  • Maintain closed files by scanning and uploading into the electronic file data base.
