About The Position

MECA Therapies is seeking a Family Service Coordinator (FSC) to join their team. As the largest pediatric rehabilitation provider in New Mexico, MECA Therapies is dedicated to ensuring families and children have access to high-quality care. The FSC will work within the Early Childhood Services department and report to the Regional Director and MECA Management Team. This role focuses on assisting families of children from birth to three years old in accessing early intervention services. The FSC will help develop Individualized Family Service Plans (IFSPs), coordinate services, ensure timely delivery, and identify additional supports for the child and family. Services will be provided in home and community settings.

Requirements

  • A bachelor’s degree in social work; counseling; psychology, nursing; special education; early childhood education or closely related field.
  • Individuals with a bachelor’s degree in another field can substitute two (2) years of direct experience in serving individuals with disabilities and/or families.
  • Proficient computer skills in Microsoft Windows.
  • Proficient organizational and record keeping skills.
  • Effective oral and written communication.
  • Excellent interpersonal and teaming skills.
  • Ability to train clients, family members and professionals.
  • Maintain a high level of confidentiality.
  • Operate within Local, State and Federal Regulations as well as adhere to all MECA policies and procedures.

Responsibilities

  • Assisting and enabling families of children to access and receive early intervention services.
  • Explaining the early intervention process, parent rights, and safeguards to families.
  • Explaining the ABA process, parent rights, and safeguards to families, as appropriate.
  • Gathering information from families regarding their concerns, priorities, and resources.
  • Coordinating evaluation and assessment activities.
  • Facilitating the determination of the child’s eligibility.
  • Connecting families with resources and supports.
  • Coordinating with medical and health providers.
  • Helping families plan and prepare for their IFSP.
  • Organizing and facilitating Transdisciplinary Team Consultation Meetings (TTCM’s) and IFSP’s.
  • Arranging and coordinating all services listed on the IFSP.
  • Monitoring the delivery of services.
  • Facilitating periodic reviews of the IFSP and ensuring that a transition plan is developed and transition conferences are held at the appropriate time.
  • Providing families with information and support to enhance their child’s development in all areas (cognitive, communication, physical/motor, social/emotional, adaptive).
  • Supporting individuals in making choices about their daily lives and finding overall purpose and direction as requested.
  • Completing Public and Private Insurance forms with each family annually to determine eligibility for Medicaid or private insurance.
  • Meeting with the family at least once a month through a face-to-face visit.
  • Managing and coordinating the transition process, from the transition plan to the transition conference.
  • Attending and facilitating monthly TTCM meetings.
  • Ensuring the initial visit is face-to-face and at least one face-to-face visit occurs each month.
  • Proving services according to the child’s IFSP.
  • Completing necessary data collection and reporting tools as mandated by MECA executive management, programs, and primary contracting agencies.
  • Creating and maintaining a confidential record for each person served.
  • Documenting all time spent with the family and work done on behalf of the family.
  • Entering all documentation into the MAPS system.
  • Completing and turning in all monthly documentation and billing submissions.
  • Completing progress notes with the team at 6-month review TTCM meetings.
  • Providing necessary support, training, and guidance to persons served.
  • Attending required Family Service Coordination trainings (online and classroom) within one year of hire.
  • Consulting, mentoring, and providing in-house trainings to other staff members as appropriate.
  • Obtaining appropriate certification through the NM FIT program.
  • Attending mandatory staff meetings and trainings.
  • Auditing a minimum of 3 client files monthly to ensure all documents are accurate and filed appropriately.
  • Acknowledging and respecting the rights of persons served at all times.
  • Ensuring all service coordination services and related activities are ethical and in compliance with MECA policies and procedures.
