About The Position

MECA Therapies is seeking a Family Service Coordinator (FSC) to join their team. As the largest pediatric rehabilitation provider in New Mexico, MECA Therapies is dedicated to ensuring families and children have access to high-quality care. The FSC will assist families of children from birth to three years old in accessing and receiving early intervention services. This role involves developing the Individualized Family Service Plan (IFSP), coordinating services, ensuring timely delivery, and identifying additional supports for the child and family. Services will be provided in home and community settings.

Requirements

  • Bachelor’s degree in social work, counseling, psychology, nursing, special education, early childhood education, or a closely related field.
  • Two (2) years of direct experience in serving individuals with disabilities and/or families can substitute for a bachelor's degree in another field.
  • Proficient computer skills in Microsoft Windows.
  • Proficient organizational and record-keeping skills.
  • Effective oral and written communication skills.
  • Excellent interpersonal and teaming skills.
  • Ability to train clients, family members, and professionals.
  • Ability to maintain a high level of confidentiality.
  • Must be bilingual (Spanish/English).

Nice To Haves

  • FSC’s are required to attend service coordination training modules Part I and II. Family Service Coordinators must complete all modules within one year of the date of hire.

Responsibilities

  • Assisting and enabling families of children to access and receive early intervention services.
  • Explaining the early intervention process, parent rights, and safeguards to families.
  • Explaining the ABA process, parent rights, and safeguards to families, as appropriate.
  • Gathering information from families regarding their concerns, priorities, and resources.
  • Coordinating evaluation and assessment activities.
  • Facilitating the determination of the child’s eligibility.
  • Connecting families with resources and supports.
  • Coordinating with medical and health providers.
  • Helping families plan and prepare for their IFSP.
  • Organizing and facilitating Transdisciplinary Team Consultation Meetings (TTCM’s) and IFSP’s.
  • Arranging and coordinating all services listed on the IFSP.
  • Monitoring the delivery of services.
  • Facilitating periodic reviews of the IFSP and ensuring a transition plan is developed and transition conferences are held at the appropriate time.
  • Providing families with information and support to enhance their child’s development in all areas (cognitive, communication, physical/motor, social/emotional, and adaptive).
  • Supporting individuals in making choices about their daily lives and finding purpose and direction.
  • Completing the Public and Private Insurance form with each family annually to determine eligibility for Medicaid or private insurance coverage.
  • Meeting with the family at least once a month through a face-to-face visit.
  • Managing and coordinating the transition process, from the transition plan to the transition conference.
  • Attending and facilitating monthly TTCM meetings.
  • Ensuring the initial visit is face-to-face and at least one face-to-face visit occurs each month.
  • Providing services according to the child’s IFSP.
  • Achieving 100% performance goal for productivity.
  • Completing necessary data collection and reporting tools as mandated by MECA executive management, programs, and primary contracting agencies (NM Department of Health and Local Education Agencies), ensuring accuracy and timely filing.
  • Creating and maintaining a confidential record for each person served, including consent and release forms, evaluation and assessment results, eligibility determination documentation, medical and other appropriate records, IFSP documents, TTA’s, progress notes, and contact notes (including date and amount of time service was provided).
  • Documenting all time spent with the family and work done on behalf of the family.
  • Entering all documentation into the MAPS system.
  • Completing and submitting all monthly documentation and billing submissions in accordance with program-specific guidelines.
  • Completing progress notes with the team at 6-month review TTCM meetings.
  • Providing necessary support, training, and guidance to persons served.
  • Complying with all mandatory trainings identified by the certification agency and by supervisors/company.
  • Attending required Family Service Coordination trainings (online and classroom) within one year of hire.
  • Consulting, mentoring, and providing in-house trainings to other staff as appropriate.
  • Obtaining appropriate certification through the NM FIT program.
  • Attending mandatory staff meetings and trainings.
  • Auditing a minimum of 3 client files monthly for quality assurance to ensure accuracy and proper filing.
  • Acknowledging and respecting the rights of persons served at all times.
  • Ensuring all service coordination services and related activities are ethical and in compliance with MECA policies and procedures.
