About The Position

Provides case management services for an assigned caseload. Works closely with individuals we serve and/or their families. Communicates with staff, vendors and the community. Organizes and resolves work-related situations.

Requirements

Bachelor's degree (B. A.) from four-year college or university; two years related experience and/or training; or equivalent combination of education and experience.
Demonstrates cultural sensitivity and awareness to diverse groups.
Must be able to handle confidential information and adhere to Welfare and Institution codes.
Must have good time management skills and be PC literate.
California Driver License, Class C with personal auto insurance (meeting minimum California requirements).

Responsibilities

Contacts assigned individuals we serve and documents contacts according to established SCLARC Standard Operating Procedures (SOPs) and policy, and completes required documentation.
Prepares case transfers as needed.
Attends all unit, division, supervision, staff and in-service training meetings as required.
Attends all Planning Team meetings at primary programs and any other individual we serve related special meetings/hearings (e.g. SCAN, court, IEPs/IFSPs staffings, mental health, governmental/community agencies, placement, etc.) as required.
Handles officer-of-the-day (OD) duties as required.
Handles confidential information and adheres to Welfare and Institution (W&I) codes.
Consults with appropriate specialists regarding the service needs of individuals we serve.
Maintains effective working relationships with top management, peers and staff.
May act as Program Manager in his/her absence.
