About The Position

As a Skill Builder II, you�ll play a vital role in helping patients and families develop essential life skills that support developmental milestones and reduce psychosocial barriers. You�ll work closely with therapists and medical staff to coordinate care and implement individualized treatment plans.

Requirements

Bachelor's degree (BA, BS, BSW) in the following disciplines: Social Work, Sociology, Psychology, Criminal Justice, Education, Nursing, Recreational Therapy, Rehabilitation, Music Therapy, Art Therapy, Pastoral Counseling, Child Development
Valid driver�s license, a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance required.

Nice To Haves

Experience in behavioral health or social services is a plus.
One to two years related experience preferred.

Responsibilities

Collaborate with therapists to develop, implement, and revise treatment plans.
Deliver individual and group skills training for adults and children, focusing on: Self-care and daily life management, Interpersonal communication and parenting, Illness and anger management, Positive coping and problem-solving skills.
Provide case management services aligned with treatment goals.
Support integrated care by coordinating with medical providers and identifying health-related needs.
Complete CANS/ANSA reassessments and maintain accurate clinical documentation.
Serve as a liaison between patients and external agencies or institutions.
Participate in ongoing professional development and training.
