We’re seeking a Shelter Case Manager who’s ready to make a difference every day. Requirements

BA/BS in Human Services preferred, along with relevant experience in human services, housing, or homelessness services.
Excellent organizational, assessment, and communication skills.
Computer proficiency with the ability to perform daily tasks within HMIS computer programs, Outlook, Microsoft Word, and Excel.
A reliable, insured vehicle and valid driver’s license required for travel between sites and community outreach.
A strong commitment to ending homelessness, exemplifying our mission with honesty, integrity, and professionalism.

Responsibilities

Help individuals achieve successful placement into stable housing by connecting them with community resources, life-skills training, job coaching, and financial empowerment.
Ensure compliance with EOHLC shelter regulations while delivering client-centered support aligned with the Housing First model.
Meet with each client weekly to evaluate progress, identify potential barriers, and develop actionable strategies.
Assist with housing applications, housing searches, job searches, budgeting goals, and obtaining essential documents like valid IDs, Social Security Cards, and Birth Certificates.
Conduct intake assessments, maintain accurate documentation, manage client case files, and track program data efficiently within HMIS computer programs.
Collaborate with the Shelter Manager and multidisciplinary teams to coordinate supportive services related to mental health, clinical issues, and substance use support.
