About The Position
Moms In Motion is seeking a Service Facilitator (SF) Case Manager to support families navigating Virginia Medicaid Waiver programs, including CCC+, CL & FIS, and EPSDT. The role focuses on helping clients live safe, healthy, and independent lives through home visits, assessments, plans of care, and ongoing guidance. The position requires living in Virginia and involves daily local travel to client homes, with remote work using a laptop or tablet. A degree is not required. Anticipated annual earnings generally range from $40,000 to $75,000 based on completing a minimum of 125 visits per month; actual earnings vary according to the number and type of visits completed.
Requirements
- Live in Virginia.
- At least 2 years of experience supporting individuals with disabilities or older adults.
- Ability to use email, documents, spreadsheets, portals, and video calls.
- Valid driver's license and reliable vehicle; client transportation is not required.
- Strong written and verbal communication skills.
- Ability to pass a background check and provide two professional references.
