About The Position

The Service Coordinator, Pathways position is a direct care role supporting mental health treatment within Sheppard Pratt’s Development and Intellectual Disabilities Services. The Supported Employment and Pathways Day Program provides clients opportunities to build community and social skills, along with individual job placement and employment support. The coordinator assesses and monitors client needs, develops service plans, connects clients with community services and resources, and advocates for clients. Scheduling is flexible and based around client needs. The position is in person in Frederick, Maryland, with a Monday–Friday dayshift schedule; availability for days, nights, weekends, and holidays may also be required depending on program needs.

Requirements

High school diploma or GED and one year of related experience.
Willingness to obtain additional certifications and training as required.
Driver’s license with three points or less and access to an insured vehicle.
Availability to work days, nights, weekends, and holidays depending on program needs.

Nice To Haves

Bachelor’s degree preferred.

Responsibilities

Assist clients with program enrollment and orientation.
Coordinate client medical appointments, transportation, housing, money management, and related needs.
Develop and assist with client medication management plans.
Obtain and maintain client entitlements.
Maintain accurate and complete documentation.
Develop and coordinate transition plans and program discharge.
Assess and monitor client needs, develop service plans, connect clients with community services and resources, and advocate for clients.
