About The Position

FIREFLY is seeking a temporary full time (1 Year) Service Coordinator to join our Coordinated Service Planning (CSP) team in either Kenora, Dryden or Red Lake. Coordinated Service Planning is designed to provide children and youth with multiple and/or complex special needs, and their families, with a seamless, family-centred service experience. As a Service Coordinator, you will lead inter-agency, multi-disciplinary care teams to develop and implement a single, integrated Coordinated Service Plan that reflects the child and family's strengths, vision, and goals. You will play a central role in ensuring services are connected, collaborative, and responsive as children grow and their needs evolve. This role requires strong leadership, maturity, and exceptional interpersonal skills in an evolving and dynamic service delivery system.

Requirements

  • Undergraduate Degree in Human Services or equivalent combination of education and experience
  • Minimum 3 years' experience working directly with children, youth, and families with multiple and/or complex special needs
  • Demonstrated experience coordinating services across multiple systems and sectors
  • Recent experience working as part of a multi-disciplinary team, ideally across agencies
  • Experience supporting families in the Kenora and Rainy River Districts
  • Experience writing integrated SMART goals involving multiple service providers
  • Demonstrated commitment to high-quality, family-centred care
  • Experience working with children and families/caregivers from diverse cultural backgrounds
  • Assessment & Planning: Strength-based, solution-focused, holistic assessment skills
  • Strong service planning and coordination skills
  • Ability to tailor service levels to child and family needs

Responsibilities

  • Engage children, youth, and families to identify their vision, strengths, and goals.
  • Develop individualized, strength-based Coordinated Service Plans.
  • Facilitate inter-agency, multi-disciplinary team meetings.
  • Lead collaborative planning processes to ensure seamless service delivery.
  • Connect families with appropriate local, regional, and provincial services.
  • Monitor progress and adjust service intensity based on changing needs.
  • Apply standardized screening tools.
  • Support conflict resolution and guide teams through competing priorities.
  • Initiate and facilitate Case Resolution processes when needs exceed locally available resources.
  • Contribute to community development by participating in and leading community networks supporting children with special needs.
  • Maintain a high standard of professional conduct and quality of care.
