About The Position

Professional Services Group is hiring a Service Coordinator for the Waukesha County Children's Long-Term Support (CLTS) program. The role provides case management and service coordination to families and individuals seeking counseling and referrals related to intellectual, developmental, physical, or mental health disabilities. The position is based at the Waukesha County Human Services building and involves local travel for client home visits, with mileage reimbursement. Hybrid or work-from-home arrangements may be available after six months.

Requirements

A bachelor's degree in sociology, social work, psychology, or a closely related field.
One year of professional social work experience.
Reliable daily transportation, a valid driver's license, and automobile insurance.

Responsibilities

Receive new client referrals and make initial contact.
Assess client needs through interviews, collateral contacts, and other investigations.
Develop casework plans with clients and provide casework services.
Maintain a caseload of clients experiencing personal, family, or socioeconomic problems, and address related protection and safety needs.
Provide counseling, intervention, and case management by coordinating social, community, and other services for clients.
Conduct in-person home visits to assess needs and maintain contact with clients.
