About The Position

Professional Services Group is hiring Case Managers for the Children's Long-Term Support (CLTS) Waiver Program in Dane County. The role provides service coordination, counseling, and referral services to youth and families affected by intellectual and/or developmental, physical, or mental health disabilities. Case Managers work with families to assess needs, maintain individualized service plans, and coordinate social, community, and other services. The position is based in Madison, Wisconsin, and requires local travel throughout Dane County for client home visits, with mileage reimbursement. The schedule is Monday through Friday, 8:00 a.m. to 5:00 p.m., with occasional evenings to accommodate families' schedules.

Requirements

• Bachelor's degree in sociology, social work, psychology, or a closely related field.
• One year of professional social work experience.
• Reliable transportation on a daily basis.
• Valid driver's license and automobile insurance.

Responsibilities

• Receive new client referrals and make initial contact.
• Assess client needs through interviews, collateral contacts, and other investigations.
• Maintain individualized service plans with clients and perform casework services.
• Manage a caseload involving clients with personal, family, or socio-economic problems, attending to related protection and safety needs.
• Provide counseling, intervention, and case management by coordinating social, community, and other services received by clients.
• Conduct in-person home visits to assess needs and maintain client contact.
