About The Position

The Child-Youth Case Manager works with children and youth in the community, engaging them in creating their own service plans and goals. The role provides social skills and behavior management support and education, and advocates for clients by connecting them with additional community supports. The Case Manager meets with clients and families in their homes, in the community, and in the office to support progress, assess needs, and adjust treatment plans as necessary.

Requirements

Bachelor’s or Master’s degree in a related field, such as Psychology or Social Work.
CPR/First Aid certification.
Valid Arizona driver’s license without restrictions, current auto insurance, and personal transportation.
Eligible for fingerprint clearance approval throughout employment.

Responsibilities

Work directly with children and youth to help create personalized service plans and goals.
