About The Position

The Service Coordinator II works in the Fulton Permanent Supportive Housing Program, which provides mental health services and care coordination to adults age 18 and older in community-based settings. The program serves people diagnosed with mental illness and substance use conditions who have experienced chronic homelessness, recent homelessness, or risk losing their housing. Its services connect members with mental health care, housing, and benefits while supporting wellness, mental health management, and community integration. The program provides 24/7 on-call coverage to support members in the least restrictive community setting possible. Its work is guided by Housing First, Mental Health Recovery, and Trauma-Informed Care. The coordinator provides clinical therapeutic and case management services, assessments, care planning, advocacy, outreach, and skill-building support. The role requires collaboration with program staff and participation in a rotating on-call schedule. The position is based in community settings and involves home visits and work with people experiencing homelessness, which may include exposure to challenging living conditions and verbal or physical expressions of anger or trauma. Staff receive training in de-escalation, and the organization has procedural safeguards intended to mitigate workplace risks.
