Requirements

  • Master’s degree in Social Work
  • Minimum of 3 years’ hands on experience in social work required
  • Demonstrated assessment skills
  • Ability to utilize problem solving theories and techniques, combined with flexibility and creativity, in working with a diverse population
  • Must possess time management skills, highly developed verbal and written skills, problem solving capacity, critical thinking, and good organizational skills
  • Requires knowledge of community resources, third party payers, and regulatory agencies
  • Currently licensed and registered in NY State to practice as a social worker (LMSW or LCSW)
  • BLS certification required upon hire, and maintained for the duration of employment
  • Must attend hospital crisis program within 3 months of hire or transfer and annually thereafter
  • Position will primarily be medium to light work requiring lifting and/or carrying objects weighing up to 50 pounds
  • Bending, twisting, stooping, kneeling, and reaching are required
  • Position requires 20% sitting and 80% standing or walking
  • Expressing or exchanging ideas by spoken and written word is required
  • Demonstrates physical and functional ability to perform full anatomical range of motion to accomplish tasks
  • Evidence of visual and aural acuity and finger and hand dexterity to operate computer and office equipment
  • Can withstand long periods of sitting, standing and/or constant walking
  • Ability to lift 50 lbs
  • Ability to foster collaborative relationships
  • Ability to work well under pressure
  • Ability to organize and synthesize new information, and prioritize tasks
  • Possesses critical thinking, analytical skills and flexibility
  • Ability to multi-task
  • Required detailed attention to work in an environment where interruptions cannot be controlled

Nice To Haves

  • 5 or more years’ preferred experience
  • Experience in the acute care/Emergency Department setting preferred
  • Competency on computer is preferred
  • May be subject to irregular hours including evenings or potentially weekends to participate in operational and community events as necessary

Responsibilities

  • Meeting the psycho-social needs of patients
  • Acting as a referral source and liaison between patients, their families and community agencies
  • Assisting the patient in optimally transitioning from one level of care to another
  • Serving as a liaison with community agencies
  • Providing consultations to medical-surgical patients
  • Screenings of Emergency Department psychiatric patients as needed
  • Providing coordination of the Social Work staff
  • Supporting their performance relative to social work assessment, discharge planning, completion of psychiatric screens, communication skills (written and verbal), participation in team conferences and unit-based rounds, group facilitation and specific daily assignments.

Benefits

  • The compensation range for the role is $41.76-$53.33 hourly.
