Under the supervision of a licensed pharmacist, this role supports high-quality, compassionate patient care while adapting to the dynamic healthcare environment. Responsibilities include obtaining and reviewing post-discharge medical records, navigating multiple electronic medical record systems (EMRs), and analyzing clinical documents such as labs, history & physicals, provider notes, and discharge summaries. The position involves conducting outreaches to members to update medication lists, coordinating with providers to confirm care relationships, and following up on clinical recommendations. Additional duties include identifying medication discrepancies, reviewing prescription claims data, and preparing detailed reports for pharmacist reviews. Candidates will collaborate across care teams to support coordination efforts and must be able to work both independently and effectively in a fast-paced, evolving setting.
