About The Position

Innovative Hematology, Inc. (IHI) is seeking a Pharmacy Nurse Care Coordinator to provide advanced clinical coordination for patients with bleeding disorders. This role involves supporting individualized treatment plans, managing complex patient cases, and collaborating with physicians and multidisciplinary teams. The coordinator will also focus on adherence monitoring, insurance navigation, and specialty-medication logistics.

Requirements

  • Valid RN or LPN license in the state of Indiana
  • ASN or BSN degree
  • All IHI employees are expected to enable multi-factor authentication via their personal smart phone/smart device in order to access IHI systems as a requirement of the role.
  • This position is open to residents of the State of Indiana, and requires routine presence at our Center in Indianapolis, IN.

Responsibilities

  • Instruct pharmacy patients on factor dosing and new product education.
  • Assess patient needs and direct patients to immediate care when necessary.
  • Provide detailed patient education on medication administration, bleed recognition, and emergency protocols.
  • Coordinate complex patient cases, facilitate triage, and ensure prompt communication of nursing assessments to providers.
  • Ensure accurate electronic medical record (EMR) documentation.
  • Cooperate and communicate effectively with pharmacy and billing departments to meet patient needs and minimize financial risks.
  • Support individualized dosing plans and factor-utilization reviews with pharmacists.
  • Identify trends in patient care and needs, and recommend/implement programs to improve patient services.
  • Monitor adherence, factor usage, and bleed tracking, including Infusion log review and education.
  • Conduct home visits and needs assessments.
  • Monitor and facilitate factor order completion and non-factor prescriptions.
  • Assist with home-infusion training and troubleshooting.
  • Assist with scheduling clinic visits, follow-ups, and telehealth appointments.
  • Support patients in understanding treatment plans and bleed management instructions.
  • Document patient interactions, refill requests, and care-coordination activities in the EMR.
  • Assist the pharmacy team in collecting clinical data (bleed logs, factor usage, adherence).
  • Maintain accurate patient records and communication logs.
