About The Position

Health at Home is seeking a compassionate and experienced Registered Nurse (RN) to serve as our Senior Living Clinical Care Coordinator within one of our partnered senior living communities. This unique hybrid role combines clinical nursing, care coordination, relationship management, and community engagement. You'll become a trusted resource for residents, families, physicians, and community staff while helping ensure seamless transitions of care and access to high-quality home health and private duty services. Requirements

  • Active Florida Registered Nurse (RN) license in good standing
  • 1 to 3 years of nursing experience in home health, senior living, care management, or a related setting
  • Excellent communication and relationship-building skills
  • Passion for serving older adults
  • Valid Florida driver's license and reliable transportation

Responsibilities

  • Serve as the on-site clinical resource for residents, families, and community staff
  • Assess residents for home health and private duty care needs
  • Coordinate care with physicians, therapists, families, and interdisciplinary teams
  • Perform skilled nursing visits, nursing assessments, and medication pours as appropriate
  • Help ensure smooth transitions of care and continuity of services
  • Develop strong relationships with residents, families, physicians, and community leadership
  • Represent Health at Home with professionalism and compassion
  • Participate in resident meetings, family conferences, and community events
  • Provide wellness education, health screenings, and informational presentations
  • Provide guidance, training, and clinical oversight for private duty caregivers
  • Assist with caregiver education and competency support
  • Help ensure quality care and exceptional client experiences
  • Maintain accurate, timely clinical documentation
  • Ensure compliance with Medicare, state regulations, and agency policies
  • Support quality improvement initiatives and patient safety standards
  • Help expand awareness of Health at Home's home health and private duty services
  • Foster strong partnerships throughout the community
  • Identify opportunities to improve resident care and service delivery
