About The Position

This role involves leadership and coordination within the HOPWA program, ensuring services meet agency and funder standards. The Senior Housing Case Manager will screen and assess clients for various housing stability services, complete applications, determine eligibility, and identify resources. They will manage an active caseload, act as a liaison for client referrals, and ensure all documentation is accurate and timely. The position also requires maintaining comprehensive knowledge of community services, supporting billing processes, and participating in staff training. Additionally, the role emphasizes a culture of service, including compassion, competency, and commitment to internal and external customers, as well as adherence to safety protocols.

Responsibilities

  • Track productivity and coordinate with the Grantee and Manager to ensure the success of the HOPWA program.
  • Assist with QA reviews and take corrective action if necessary.
  • Ensure services provided by Housing Case Managers meet or exceed agency and funder standards.
  • Review Housing Case Manager’s documentation and billing for timeliness, accuracy, legibility, and clarity.
  • Screen and/or assess all new clients for HOPWA services and other forms of housing stability services according to the intake schedule.
  • Complete applications for HOPWA programs and other housing programs.
  • Determine eligibility for HOPWA programs and other housing programs.
  • Identify resources and refer to appropriate housing programs.
  • Follow up on clients’ housing stability (HOPWA and non-HOPWA housing).
  • Maintain an average annual active caseload as assigned by Management.
  • Act as a liaison when required to ensure that clients are properly referred within agency programs or external services.
  • Ensure all documentation is timely, accurate, legible, and clear.
  • Empower clients to participate in their treatment planning as needed.
  • Maintain progress notes and progress reviews in client records as specified in agency policy, program guidelines, and performance standards.
  • Input client information using electronic data entry according to agency and departmental guidelines.
  • Maintain an accurate record on the time sheet reflecting time spent in the program worked.
  • Prepare necessary program reports and records as requested by the supervisor and/or manager.
  • Maintain comprehensive knowledge of community services to apply knowledge of services to individual client needs.
  • Support billing through concurrent documentation of service provided and budget activities.
  • Participate in staff training sessions within the timeframes specified and as required by the agency and the funding source.
  • Participate in agency developmental activities as requested.
  • Ensure proper hand washing according to Centers for Disease Control and Prevention guidelines.
  • Understand and appropriately act upon assigned role in Emergency Code System.
  • Understand and perform assigned roles for the health center Continuity of Operations Plan (COOP).
  • Greet internal or external customers with courtesy, making eye contact, and responding with a proper tone and nonverbal language.
  • Listen to internal or external customers attentively, reassuring and understanding of the request and providing appropriate options or resolutions.
  • Provide services required by following established protocols and when needed, procure additional help to answer questions to ensure appropriate services are delivered.
  • Take initiative and anticipate internal or external customer needs by engaging them in the process and following up as needed.
  • Prioritize internal or external customer requests to ensure a prompt and effective response is provided.
  • Other duties as assigned.
