About The Position

St. Mary's General Hospital is seeking a Senior Case Manager for adolescent services. Depending on the program and the individual’s changing needs, the role provides community-based mental health case management with varying degrees and scopes of intervention. In adolescent partial care, the position provides direct care, including individual and group counseling, support, and other activities. The role also contributes to assessment, treatment and discharge planning, clinical documentation, family and collateral meetings, team meetings, and case conferences to support continuity of care.

Requirements

Master’s degree from an accredited college or university in psychology, social work, or rehabilitation.
Two years of mental health experience.
Bachelor’s-level candidates need three years of relevant experience, including at least one year in a mental health case management position.
Comprehensive understanding of mental health services and the role of case managers in mental health services.
Valid New Jersey driver’s license.
Bachelor’s degree candidates currently enrolled in a graduate program and with additional experience may be considered on a case-by-case basis.

Responsibilities

Provide community-based mental health case management, with the scope of intervention responsive to the program and individual needs.
Provide direct care services, including individual and group counseling, support, and other activities.
Participate in comprehensive assessments and the development of treatment and discharge plans.
Maintain clinical documentation.
Participate in and facilitate family and collateral meetings, team meetings, and case conferences to ensure continuity of care.
