Established in 1975, Non-Profit Yakima Neighborhood Health Services serves patients in the areas of Behavioral Health, Family Dentistry, Internal Medicine, Women’s Health, Primary Care, Pediatrics, Vision Care and Pharmacy. Our mission is to improve the quality of life and equity in our communities by providing accessible and integrated health and social services, ending homelessness and offering unique learning opportunities for students of health professions. This role involves understanding and overseeing Foundational Community Support programs – specifically Washington State Supportive Housing (SH) and Supported Employment (SE) programs. The supervisor will provide training, supervision, and monitoring of staff performing SH and SE activities, including eligibility, referral, and coaching procedures, using evidence-based principles and practices. They will also monitor billing activities of SE/SH Specialists, working with the Revenue Cycle Manager to assure appropriate billing procedures, and provide FCS program feedback to team members. Additionally, the role includes outreach to private and public landlords to make rental units available for homeless individuals and families, educating them on Permanent Supportive Housing, Housing First, and Trauma Informed Care. Outreach to potential employers to identify employment opportunities for Supported Employment participants is also a key responsibility. The supervisor will act as a liaison to other departments and organizations to coordinate services, provide SE/SH services directly to participants, and participate in reporting activities. Collaboration with homeless agencies and social service providers to address patient needs is essential. The role requires maintaining an organized, clean, efficient, and confidential work area and representing YNHS in local meetings as requested. Performing related work and other tasks as requested is also part of the job.
