About The Position

The Center for Human Development (CHD) is seeking a part-time School-Based Community Health Worker to join our Community School Based Clinic based in Franklin County. The School Based Tele Behavioral Health Program provides students with year-round long-term or short-term counseling via tele and community resources. The Community Health Worker will work alongside a Clinician and school personnel to help students and families access services and resources. They will provide case management, ensuring students and families receive the help they need. They will document encounters, collect data, and complete monthly reports. They will support with making referrals, obtaining releases of information, consents, and other medical documentation.

Requirements

  • High School Diploma or Equivalent required
  • Strong computer skills- Electronic medical record (EMR) databases.
  • Knowledge of area community agencies and resources.
  • Skill in organizing resources and establishing priorities.
  • Creative and analytical thinking.
  • Strong interpersonal and communication skills.

Responsibilities

  • Provide home, school and community-based care coordination to youth and their families
  • Assist students and families to access on site tele behavioral counseling services as well as offsite community-based clinicians and non-clinical resources (as needed).
  • Conduct comprehensive screening/assessment activities including mental health status, health screen and social needs factors, to support community-based referrals.
  • Provide ongoing follow-up, motivational interviewing and goal setting with students/families.
