About The Position

As a School-Based Behavioral Health Patient Navigator (BH-PN) at Hawaiʻi Island Community Health Center (HICHC), you'll play a crucial role in assessing students' needs, developing service plans, and collaborating with community resources to ensure students receive the best care possible.

Requirements

  • A Bachelor's degree in a relevant human services field plus at least one year of experience, or an equivalent combination of experience, training, and education.
  • Previous experience working in a team environment and knowledge of the Hawaiʻi Island community.
  • Eligibility for future licensure in Social Work, Counseling, or a related field.
  • Strong social skills, problem-solving abilities, and leadership qualities.
  • Excellent communication skills and the ability to work as part of an interdisciplinary team.
  • Physical stamina to stand, walk, lift up to 50 pounds, and perform other physical tasks.

Responsibilities

  • Screening new service requests and ensuring students are served according to their individual needs.
  • Working directly with students to reduce barriers and address social determinants of health (SDOH).
  • Educating students on SDOH and helping them navigate various systems.
  • Developing treatment/service plans and establishing alliances with students.
  • Providing ongoing support and case management to help students achieve their goals.
  • Participating in specialized support services like Education and Wellness groups.
  • Collaborating with school staff to integrate health education and social-emotional wellness programming.
  • Engaging in interagency planning and service coordination to improve service continuity.
