About The Position

Greenwell Health is seeking a compassionate and detail-oriented Scheduling & Intake Coordinator to join our Team. In this role, you will provide exceptional customer service to potential patients and their families, professional referral sources, and colleagues. You will efficiently coordinate referrals and respond to inquiries about our services, building and maintaining strong provider relationships.

Requirements

  • High School Diploma required
  • Minimum of two years of experience in an office setting
  • At least one year in a healthcare, ideally hospice agency office, preferred.
  • Proficiency with computer use and software applications, including MS Office products.
  • Ability to work varying hours, including weekends, nights, and holidays.
  • Uses limited independent judgment to make decisions based on precedents and established guidelines.
  • Solves problems using standard procedures and precedents.
  • Knows when to refer issues to a supervisor and when to handle them personally.
  • Working knowledge of basic analytic abilities and overall understanding of the work environment and processes.
  • Excellent oral and written communication skills
  • Exceptional customer service skills
  • Strong organizational skills.
  • Proficiency with computers and software applications such as Microsoft Office.

  • Associate/Bachelor degree preferred
  • Experience with EMR software preferred

Responsibilities

  • Receive professional referrals from discharge planners, physician offices, and facility staff.
  • Obtain and process critical referral information through various methods (electronic/fax notification, phone call, Care Coordinator (CC) notification) to ensure timely response and data entry into our EMR system.
  • Handle inquiries from potential patients and their families regarding our services.
  • Gather and input basic patient information to describe how Greenwell Health can assist, aiming to schedule a visit to initiate the relationship in the EMR system.
  • Coordinate with Admissions staff to ensure appropriate staff make initial contact based on patient condition and family situation, meeting the needs of both the professional referral source and the patient/family.
  • Maintain clear communication with referral sources, attending physicians, colleagues, and other agencies as needed to coordinate the initiation of hospice services.
  • Ensure seamless coordination of information and services through regular communication during the sign-on, consent, and admission assessment phases.
  • Maintain referral and admissions statistics per protocol, follow up on all pending referrals, and document patient status as needed.
  • Build and maintain close professional relationships with referral sources, commercial insurance providers, and local third-party insurers through timely written and verbal communication.
  • Uphold HIPAA confidentiality of sensitive patient health care information.
  • Participate in orientation for new CCs, cross-training, and ongoing education programs.
  • Demonstrate familiarity with organizational policies and state/federal regulations related to hospice services.
  • Engage in committees, special projects, and other related duties as assigned.
