Schedule Coordinator Home Health - Full-time
About The Position
The Schedule Coordinator schedules patients with field clinicians and communicates with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care. The role also manages the on-call notebook and hospitalization logs to support communication among stakeholders. Except for designated full-time remote positions, in-office attendance is essential to support supervision, teamwork, task completion, and other position objectives. Occasional remote work may be permitted for temporary personal needs or crises beyond the company’s control.
Requirements
- High school diploma or equivalent.
- Licensed practical nurse or licensed vocational nurse.
- Maintain a valid clinical license according to state requirements and applicable board rules.
- At least one year of clinical experience in a healthcare setting, or at least one year of home health, hospice, or pediatric experience within the last 24 months with demonstrated understanding of staffing and scheduling requirements related to home care services.
- Maintain CPR certification as required by the position.
- Maintain a valid state driver license in the current state of residence.
- Maintain automobile liability insurance as required by law.
- Basic demonstrated technology skills, including operation of a mobile device.
- Dependable transportation in good working condition and ability to drive safely in all types of weather.
- Oral and written communication skills, fluency in English, and active listening.
- Intermediate technology skills, especially Microsoft Office, including Word and Excel.
- In-office attendance is essential except for designated full-time remote positions.
Nice To Haves
- Previous experience using Homecare Homebase (HCHB).
- Bilingual English/Spanish ability to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
