Schedule Coordinator Home Health - Full-time
About The Position
The Schedule Coordinator Home Health is responsible for scheduling patients with field clinicians and coordinating care-related communication. The role communicates with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care. It also manages the on-call notebook and hospitalization logs to support communication among stakeholders. In-office attendance is essential except for designated full-time remote positions; occasional remote work to address temporary personal needs or crises does not waive this requirement.
Requirements
- High school diploma or equivalent.
- Licensed practical or vocational nurse.
- Maintain a valid clinical license in accordance with state requirements and applicable board rules.
- At least one year of clinical experience in a healthcare setting, or at least one year of home health, hospice, or pediatric experience within the last 24 months, with demonstrated understanding of home-care staffing and scheduling requirements.
- Maintain CPR certification, as required by the position.
- Maintain a valid state driver's license in the current state of residence.
- Maintain automobile liability insurance as required by law.
- Basic demonstrated technology skills, including operating a mobile device.
- Dependable transportation in good working condition and ability to drive safely in all types of weather.
- Oral and written communication skills, fluency in English, and active listening.
- Intermediate technology skills, especially Microsoft Office, including Word and Excel.
Nice To Haves
- Previous experience using Homecare Homebase (HCHB).
- Bilingual English/Spanish ability to facilitate communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
