About The Position

Enhabit Home Health & Hospice is seeking a Schedule Coordinator to schedule patients with field clinicians and help coordinate continuity of care. The position communicates with field staff, patients, physicians, referral sources, caregivers, and other service providers, and manages the on-call notebook and hospitalization logs. In-office attendance is essential except for designated full-time remote positions; occasional remote work to accommodate temporary personal needs or crises does not waive this essential function.

Requirements

• High school diploma or equivalent.
• Licensed practical or vocational nurse.
• Maintain a valid clinical license according to state requirements and applicable board rules.
• At least one year of clinical experience in a healthcare setting, or at least one year of home health, hospice, or pediatric experience within the last 24 months with demonstrated understanding of home-care staffing and scheduling requirements.
• Maintain CPR certification, as required by the position.
• Maintain a valid driver license in the current state of residence.
• Maintain automobile liability insurance as required by law.
• Basic demonstrated technology skills, including operating a mobile device.
• Dependable transportation in good working condition and ability to drive safely in all types of weather.
• Oral and written communication skills, fluency in English, and active listening.
• Intermediate technology skills, especially Microsoft Word and Excel.

Nice To Haves

• Previous experience using Homecare Homebase (HCHB).
• Bilingual English/Spanish ability to communicate clearly with patients and caregivers.
