About The Position

Masonicare Home Health & Hospice, an affiliate of Masonicare, is seeking a Registered Nurse Case Manager serving the Middletown, Connecticut area. The role ensures the quality and safe delivery of nursing services to patients in their homes. Visits are primarily scheduled during the day, with flexible scheduling, and a weekend rotation is required—typically one weekend per month or less. The nurse develops, implements, and evaluates patients’ plans of care and makes revisions to address identified problems. The role coordinates care with other health professionals, maintains continuity of care, identifies interdisciplinary needs, and recommends additional home health services. Visiting Nurses supervise care delivered by Licensed Practical Nurses and Home Health Aides for delegated duties and conduct supervisory visits in accordance with regulatory guidelines. Masonicare uses electronic documentation. Nurses are expected to document patient and job information accurately and promptly to support OASIS data, service orders, timesheets, and other required patient and employment documents and reports. Requirements

Minimum of 1 year of clinical experience; new graduates will not be considered.

Nice To Haves

Previous home care experience is desired but not required.

Responsibilities

Ensure the quality and safe delivery of nursing services to patients in the home setting.
