About The Position

Spring Valley Hospice, an affiliate of Adoration Health, is seeking a Hospice Registered Nurse Case Manager to provide direct care and coordinate with the interdisciplinary team so patients and families receive an optimal care experience. The role supports hospice patients through a holistic approach focused on managing pain and other symptoms while providing emotional and spiritual support to families. Spring Valley Hospice offers routine home care, respite, general inpatient care, and continuous care.

The position follows a Monday–Friday schedule from 8 a.m. to 5 p.m. On-call requirements may vary.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least 1 year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families.
• Initiate patient plans of care and reevaluate them as necessary.
• Use health assessment data to determine nursing diagnoses.
• Administer medications and treatments as prescribed by the physician in the plan of care.
• Counsel patients and families on meeting nursing and related needs.
• Complete and maintain accurate clinical notes.
• Communicate with physicians and participate in interdisciplinary group meetings to address the medical and emotional needs of patients and their families or caregivers.
• Ensure compliance with state and federal legal and regulatory requirements.
• Perform additional responsibilities as assigned.
