About The Position

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide individualized, holistic hospice care to patients in their homes. The role is a cornerstone of patient care, working with a care team to develop personalized plans of care, lead end-of-life conversations, and provide compassionate, clinically excellent symptom management. The position is based in Oak Creek, Wisconsin, and involves 100% travel. It is full-time, scheduled from 8:00 a.m. to 5:00 p.m., with on-call expectations.

Requirements

• Associate, bachelor's, or master's degree from an accredited nursing program (RN).
• At least 1 year of recent acute care experience.
• Reliable transportation, a valid driver's license, and proof of auto insurance.

Nice To Haves

• Hospice or home health experience is preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families, updating them as needs evolve.
• Administer prescribed medications and treatments.
• Educate patients and caregivers about care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
• Collaborate with the care team to provide individualized, holistic patient support.
• Lead end-of-life conversations and support effective symptom management.
