About The Position

Spring Valley Hospice, an affiliate of Adoration Health, is seeking a Registered Nurse Case Manager to provide direct hospice care and coordinate with the interdisciplinary team so patients and their families receive an optimal care experience. The role focuses on managing patients’ pain and other symptoms while providing emotional and spiritual support to families. Spring Valley Hospice offers routine home care, respite, general inpatient care, and continuous care, with a holistic approach that values quality of life as much as length of life.

The nurse completes comprehensive patient and family assessments, develops and reevaluates plans of care, administers prescribed medications and treatments, counsels patients and families, and maintains accurate clinical documentation. The RN also communicates with physicians, participates in interdisciplinary meetings, and ensures compliance with applicable state and federal requirements.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least 1 year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families.
• Initiate patient plans of care and reevaluate them as necessary.
• Use health assessment data to determine nursing diagnoses.
• Administer medications and treatments as prescribed by the physician in the plan of care.
• Counsel patients and families in meeting nursing and related needs.
• Complete and maintain accurate clinical notes.
• Communicate with physicians and participate in interdisciplinary group meetings to address patients’ and families’ or caregivers’ medical and emotional needs.
• Ensure compliance with all state and federal legal and regulatory requirements.
• Perform additional responsibilities as assigned.
