About The Position

As a member of the Home Health interdisciplinary team, this role assists patients and their families with care issues resolutions via telephone, fax, and epic in-basket communications. The position coordinates the delivery of care with Physicians, therapy, and nursing disciplines. It also provides education and instruction to patients and their families via telephone communications.

Requirements

  • Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)
  • College Diploma: Nursing
  • 2yr of Nursing experience

Responsibilities

  • Performs patient care over telephone or Vidyo in a timely manner
  • Identifies patient orders and works to prevent procedure delays assuring continuation of services.
  • Completes supply ordering utilizing department formulary.
  • Ensures timely processing of open orders for all clinical procedures.
  • Manages telephonic visits to promote health of clients.
  • Efficiently utilizes visits to manage cost per episodes.
  • Completes documentation, meets current standards along with policies and procedures including documentation of all mandatory forms within the scope of the position.
  • Anticipates deteriorating life signs patterns and changes in patient condition and takes appropriate action.
  • Documents phone encounters accurately in the medical record.
  • Utilizes phone etiquette skills and performs service recovery methods to ensure high customer satisfaction.
  • Manages Epic in-basket communications to facilitates prompt communication between Home Health clinicians and physician’s offices
  • Notifies Manager or designee of any issues impacting flow in a timely manner for resolution development.
  • Maintains Accurate detailed reports and records with regards to preventing and presenting complaints.
  • Works to maintain a good rapport and cooperative working relationship with physician, departments, staff, patients and caregivers.
  • Provides evidence-based practices and standards of clinical care to educate patients and patient’s representatives.
  • Collaborates with patient to identify treatment plan goals. Encourages family participation as appropriate.
  • Assists clinical employees to gain knowledge about services excellence principles, Joint Commission, CMS, regulatory requirements and clinical documentation.
  • Works with scheduler to coordinate additional patient visit needs.
  • Collaborates with intake team to provide information received regarding intake information via phone.
