About The Position

This position is responsible for planning, directing, coordinating, and providing high-quality, individualized patient care using the components of the nursing process. The role involves performing duties in a manner that provides for the physical, psycho-social, and spiritual needs of critically ill patients. This includes completing initial assessments and ongoing reassessments, incorporating cultural and ethnic factors, differentiating between normal and abnormal findings, and considering functional/developmental age for adolescent through geriatric patients. The nurse will demonstrate sound clinical judgment in developing, implementing, and evaluating the nursing aspects of the trans-disciplinary care plan, collaborating with patients/families to create individualized care plans and interventions. The role also involves evaluating patient outcomes, establishing priorities for patient care services, accurately documenting patient care activities, and utilizing a case management approach to coordinate care across the continuum. Patient safety, patient/family teaching, and providing care in emergency situations are key responsibilities. The nurse must maintain knowledge of therapeutic patient care issues and interventions, ensure patient rights are respected, determine the presence of Advance Directives, and involve patients/families in decision-making. Reporting suspected cases of abuse or neglect is also required. The role demands knowledge of age-specific care for adult and geriatric populations, including safety measures, physiological values, skin integrity, behavior, motor skills, and effective communication. The nurse will identify patient/family educational needs, initiate and provide individualized teaching, and evaluate its effectiveness. Demonstrating effective Team Leader/Charge Nurse skills, practicing Universal Precautions, and showing flexibility in changing assignments, staffing levels, and patient care environments are essential. The nurse must also complete work on time.

Requirements

  • Graduate of a RN Training Program
  • Minimum one-year recent experience in an acute care setting preferred
  • Ability to communicate effectively verbally and in writing.
  • Must be able to work in a union environment.
  • Current California RN license
  • AHA BLS card
  • AHA ACLS card
  • EKG certification
  • NIHSS Certification
  • Current Los Angeles County Fire Card required (within 30 days of hire).
  • Assault Response Competency (ARC) (within 30 days of hire).

Responsibilities

  • Planning, guiding, directing, and coordinating overall aspects of patient care across the continuum utilizing a case management approach.
  • Effectively communicating with physicians, staff, and other disciplines.
  • Assessing and responding appropriately to ensure patient safety based on the physical environment, equipment, and the patient’s behavior.
  • Participating in patient/family teaching by reviewing educational materials and coaching nurses and hospital staff on education interventions.
  • Demonstrating the ability to provide care in emergency situations and following established emergency procedures and practices.
  • Communicating and following the organizational chain of command for notification of patient care/service issues, when appropriate.
  • Maintaining knowledge of, and demonstrating the ability to identify therapeutic patient care issues/problems and determining measures and interventions to promote, maintain and/or restore health for the adolescent through geriatric patient population.
  • Providing appropriate care through the healthcare continuum including the end of life.
  • Assuring that the rights of all patients are respected and maintained by allowing for privacy and dignity in the provision of patient care.
  • Determining the presence and type of Advance Directive, if applicable.
  • Ensuring patient and/or family involvement in the decision-making process as it relates to patient care, end of life, and ethical issues.
  • Reporting suspected cases of abuse or neglect relative to the adolescent through geriatric patient population as appropriate.
  • Demonstrating knowledge of age specific care for adult and geriatric populations, including safety measures, physiological normal values/readings, and assessing skin integrity, behavior, motor skills, and/or activities that place patients at risk.
  • Communicating effectively in a clear, concise, understandable manner.
  • Identifying patient/family educational needs; initiating and providing individualized teaching which considers the functional/developmental age and the needs of the adult patient population.
  • Evaluating and documenting the effectiveness of patient/family teaching through assessment of their comprehension of the instruction and demonstration of appropriate responses.
  • Demonstrating knowledge of age specific care for the adolescent through geriatric population, including safety measures, equipment use; physiological normal values/readings; and assessing skin integrity, behavior, motor skills and/or activities that place patients at risk.
  • Demonstrating effective Team Leader/Charge Nurse skills.
  • Practicing Universal Precautions.
  • Demonstrating flexibility in the work setting with changing patient’s assignments, varying staffing levels and patient care environments.
  • Completing work on time.
