About The Position

The Team Captain RN and Triage Nurse RN are positions that allow for a collaborative approach to the patient's healthcare in a multitude of ways. The Team Captain RN is responsible for the overall care of patients assigned to their team's providers, including risk assessment, coordination of care for high-risk patients, tracking quality metrics, delivering patient education, and serving as the primary point of contact for patients and external care providers. This role supervises and directs the activities of team nurses and support staff. The Triage Nurse RN engages in an interactive process with patients, either over the phone or face-to-face, to identify the nature and urgency of healthcare needs and determine the appropriate disposition. This role involves patient assessment, education, and crisis intervention, aiming to direct patients to the correct level of care or service safely and efficiently, while also providing self-care advice.

Requirements

  • Registered Nurse required; with current license to practice in the State of Indiana
  • Four years progressive professional experience as a nurse required
  • Must complete Eskenazi Institute for Leadership training within 18 months of assignment.

Nice To Haves

  • One-year previous leadership experience strongly preferred.
  • BSN preferred.

Responsibilities

  • Supervises and directs the activities of team nurses and support staff.
  • Assesses patients, provides patient education, and crisis intervention.
  • Determines the appropriate disposition for patient healthcare needs.
  • Innovates, takes risks, and experiments to improve outcomes.
  • Manages timelines/deadlines of team projects and site projects as assigned.
  • Signs off on staff competencies.
  • Participates in interview process and evaluations for clinical positions.
  • Acts as an active listener to patients, families, and staff members in conflict.
  • Remains calm in conflict situations and works to seek mutually beneficial solutions.
  • Promotes practice growth through attracting and retaining new patients.
  • Ensures timely completion of requests for medication refills, test results, and other patient concerns.
  • Works patient panels to meet quality metrics by outreaching to patients needing disease- or age-specific screening and testing.
  • Works with team providers and members to educate patients on diagnoses, medications, wellness, and self-management skills.
  • Provides nurse visits for assigned patients and ensures team members provide care within their scope of practice.
  • Demonstrates a self-directed work effort aligned with team goals.
  • Works collaboratively in the care and care management of patients and their families.
  • Demonstrates clinical nursing competence, understanding of job expectations, and compliance with organizational policies and procedures.
  • Accountable for the safety and care of patients/families within the team's panel.
  • Functions as a resource to the team and others within the health center and EHC.
  • Uses critical thinking skills in patient care and to resolve team issues.
