Home Care Network (HCN) is seeking a highly skilled, experienced, and accountable RN Supervisor interested in building a long-term employment relationship with our agency, clients, staff, and care team partners. This is a Full-Time, leadership position to manage care throughout our Northeast Ohio region, serving approximately 27 counties, with a central focus in Summit, Stark, and Tuscarawas counties. This role requires a mix of Nurse Supervisory services and recertification visits, and travel, as needed to support all clients and staff across Northeast Ohio. The role works closely with and reports directly to our NE OH RN Program Administrator and Director of Clinical & Administrative Operations. Work consists of case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. Home Care Network, Inc. (HCN) is a Medicare-certified home care and home health agency specialized in total patient & family support for Medicaid-eligible patients including infants, pediatrics, adults, and seniors. We are nurse-owned and locally led, meaning our patients' care, trust, and wellbeing - not profits - come first. For our employees, it also means that our agency is an environment nurses and caregivers thrive in, allowing us to handpick the best care team members to be your peers and delight our clients. The result is that HCN nurses and aides benefit from being surrounded by competent and happy peers, satisfied clients, and competitive pay that promotes both personal and professional growth. Together, we can empower our patients and their families with the independence they deserve - while you make a difference in an agency where you matter. This position requires travel to approximately 27 counties in Northeast Ohio but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). At HCN, this is a field staff leadership role and the right RN Supervisor has a passion for delivering consistently excellent care and care experiences to clients, and the ability to do so directly and through the thoughtful leadership of others.
