About The Position

Home Care Network (HCN) is seeking a highly skilled, experienced, and accountable RN Supervisor interested in building a long-term employment relationship with our agency, clients, staff, and care team partners. This is a Full-Time, leadership position to manage care throughout our Northeast Ohio region, serving approximately 27 counties, with a central focus in Summit, Stark, and Tuscarawas counties. This role requires a mix of Nurse Supervisory services and recertification visits, and travel, as needed to support all clients and staff across Northeast Ohio. The role works closely with and reports directly to our NE OH RN Program Administrator and Director of Clinical & Administrative Operations. Work consists of case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall. Home Care Network, Inc. (HCN) is a Medicare-certified home care and home health agency specialized in total patient & family support for Medicaid-eligible patients including infants, pediatrics, adults, and seniors. This position requires travel to approximately 27 counties in Northeast Ohio but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). Requirements

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • At least 1 year of experience is required in: Nursing Supervision; wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent, reliable approach to all work.
  • Willingness and ability to perform required physical demands.
  • Consent + Ability to pass a State & Federal background check (including fingerprints).
  • 18yo + active Driver's License / State ID + US Work Authorization.
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).

Responsibilities

  • Coordinate, re-evaluate and confirm the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.
  • Oversee patient services and provide expert nursing care with core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning.
  • Submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • Ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client and his/her family.
  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to care team.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
