About The Position

Home Care Network (HCN) is seeking a highly skilled, experienced, and accountable RN Supervisor to manage care throughout our Northeast Ohio region, serving approximately 27 counties. This is a Full-Time, leadership position that involves a mix of Nurse Supervisory services, recertification visits, and travel to support clients and staff. The role works closely with and reports directly to the NE OH RN Program Administrator and the Director of Clinical & Administrative Operations. HCN is a Medicare-certified home care and home health agency specialized in total patient & family support. Being nurse-owned and locally led, HCN prioritizes patient care, trust, and wellbeing. Requirements

  • At least 1 year of experience in Nursing Supervision.
  • At least 1 year of experience in wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments.
  • Excellent documentation and field staff leadership skills.
  • At least 1 year of paid experience at/with a home health agency or in-home care provider (or equivalent).
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date).
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent, reliable approach to all work.
  • Familiarity with HIPAA regulations and medical terminology.
  • Willingness and ability to perform required physical demands.
  • Consent + Ability to pass a State & Federal background check (including fingerprints).
  • 18yo + active Driver's License / State ID + US Work Authorization.
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).

Nice To Haves

  • 2-5 years' experience at/with a home health agency or in-home care provider is preferred.

Responsibilities

  • Manage care throughout the Northeast Ohio region, serving approximately 27 counties.
  • Perform Nurse Supervisory services and recertification visits.
  • Travel as needed to support all clients and staff across Northeast Ohio.
  • Conduct case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services.
  • Conduct new hire orientations, in-service and supervisory reviews.
  • Provide input to influence training and education, care practices and outcomes.
  • Coordinate, re-evaluate, and confirm the medical necessity of continued home health care services.
  • Oversee patient services and provide expert nursing care.
  • Submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • Ensure successful communication with and between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and his/her family.
  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to care team.
  • Promote personal safety and a safe environment for clients and coworkers.
