About The Position

Home Care Network (HCN) is seeking a highly skilled, experienced, and accountable RN Supervisor for a Full-Time leadership position to manage care throughout their Northeast Ohio region, serving approximately 27 counties with a central focus in Summit, Stark, and Tuscarawas counties. This role involves a mix of Nurse Supervisory services, recertification visits, and travel to support clients and staff. The position reports to the NE OH RN Program Administrator and the Director of Clinical & Administrative Operations. HCN is a nurse-owned and locally led Medicare-certified home care and home health agency focused on patient and family support. They emphasize a positive work environment for nurses and caregivers, aiming to empower patients and families with independence.

Requirements

  • At least 1 year of experience in Nursing Supervision.
  • At least 1 year of experience in wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments.
  • Excellent documentation and field staff leadership skills.
  • At least 1 year of paid experience at/with a home health agency or in-home care provider (or equivalent).
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date).
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent, reliable approach to all work.
  • Familiarity with HIPAA regulations and medical terminology.
  • Willingness and ability to perform required physical demands.
  • Consent + Ability to pass a State & Federal background check (including fingerprints).
  • 18yo + active Driver's License / State ID + US Work Authorization.
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).

Nice To Haves

Responsibilities

  • Manage care throughout the Northeast Ohio region, serving approximately 27 counties.
  • Perform Nurse Supervisory services and recertification visits.
  • Travel as needed to support all clients and staff across Northeast Ohio.
  • Coordinate, re-evaluate, and confirm the medical necessity of continued home health care services.
  • Provide patient care and ensure optimal outcomes.
  • Oversee patient services and provide expert nursing care.
  • Submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • Ensure successful communication with and between physicians, agency staff, service administrators, other caregivers and clinicians, and the client and their family.
  • Perform client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document/Communicate/Coordinate patient status/change in condition to the care team.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
