About The Position

This role is responsible for coordinating all aspects of the Stroke Program to achieve and maintain Joint Commission Certification for Community Medical Centers (CMC). The coordinator will manage stroke patient care across the continuum in collaboration with unit leadership, provide stroke-related training and competencies for staff, and coordinate community outreach. The position also involves supporting monthly interdisciplinary meetings and quality assurance and performance improvement activities for the stroke program.

Requirements

  • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
  • 2 years of relevant experience in the healthcare industry or a healthcare setting required
  • RN - Current and valid Registered Nurse license to work within the state of California required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • One of the following is required within 1 year of hire or transfer into position: SCRN - Stroke Certified Nurse, CNRN - Certified Neuroscience Nurse, or ASC - Advanced Stroke Coordinator

Nice To Haves

  • Bachelor's Degree in Nursing or Healthcare related field preferred
