About The Position

As a Home Health Case Manager, the Registered Nurse delivers hands-on patient care in patients’ homes and coordinates services provided by other healthcare providers. The role communicates with health insurance companies, oversees visiting nurses, and develops personalized care plans based on physician orders and thorough initial assessments. The nurse collaborates with fellow nurses and professionals in Physical Therapy, Speech Therapy, and Occupational Therapy. Nurses complete charting during visits with the support of Roger’s AI-powered system.

Requirements

• Missouri or Compact Registered Nurse license.
• Driver’s license.
• Vehicle with current auto insurance.
• Strong background in physical assessment, diagnostic interpretation, and effective communication.

Nice To Haves

• Home Health or Critical Care experience preferred.

Responsibilities

• Manage a caseload, including supervising LPN visits.
• Conduct routine visits, evaluations, and starts of care as part of a varied weekly caseload.
• Educate patients on health conditions and care, including CHF, COPD, diabetes, wound care, and medication management.
• Communicate with physicians to obtain orders and write diagnosis-driven plans of care.
• Coordinate services of other healthcare providers and communicate with health insurance companies.
• Oversee the work of visiting nurses.
• Collaborate with nurses and Physical Therapy, Occupational Therapy, and Speech Therapy professionals.
• Complete charting during patient visits using Roger’s AI-powered support.
